Our Framework for Functional Health, Mobility & Independence – Live Stonger. Move Better. Stay Independent.
At Cogent Rehab in Burlington, we use a functional health and wellness care framework to support adults and seniors living with long-term health conditions. This approach focuses on improving strength, balance, endurance, mobility, and confidence through safe, progressive physiotherapy and ongoing wellness support.

Our Senior Care & Wellness Program is built directly on this framework—providing specialized support for adults 55+ while remaining rooted in the same evidence-based chronic disease principles.
What Is Functional Health & Wellness Care?
Functional health and wellness is based on the Chronic disease management framework focused on supporting people living with long-term conditions such as:

-
Arthritis (hip, knee, spine, hands)
-
Diabetes and neuropathy
-
Heart disease and high blood pressure
-
Chronic pain and deconditioning
-
Osteoporosis and fracture risk
-
COPD or reduced lung capacity
-
Parkinson’s disease and neurological conditions
-
Post-stroke mobility changes
Our goal is simple:
Help you move with less pain, improve strength and balance, prevent decline and maintain confidence.
How Physiotherapy Helps with Chronic Conditions?
Your care is one-on-one, personalized, and focused on meaningful daily-life improvements. Treatment may include:

How Senior Care & Wellness Fits into Chronic Disease Management
Most chronic conditions become more challenging with age. That’s why our Senior Care & Wellness Program integrates directly into chronic disease care.

This program is ideal if you or a loved one is managing any of the following:
-
Arthritis and joint stiffness
-
Balance problems or fall risk
-
Heart disease, high blood pressure, or post-cardiac rehab
-
Diabetes-related weakness or neuropathy
-
Parkinson’s or neurological changes
-
Difficulty with walking, stairs, or daily activities
-
Fatigue or mobility decline after hospitalization
The Seniors Wellness Physiotherapy Program provides:
-
Safe, progressive exercise designed for older adults
-
Fall-prevention & balance retraining
-
Gait practice and mobility confidence
-
Cardiac-safe conditioning (including Phase 3 support)
-
Neurological movement training (amplitude, cueing, dual-tasking)
-
Functional training for real life (stairs, reaching, sit-to-stand)
This makes Cogent Rehab's Senior Wellness the chronic disease pathway designed specifically for adults 60+.
How is the Functional Health & Wellness Program Delivered?
The program is delivered through Safe, Therapist-Guided Circuit Training. To help you stay consistent and continue progressing, we offer a supervised, physiotherapist-designed circuit that improves strength, balance, cardiovascular endurance, mobility and confidence
Circuit stations will include exercises and activities targeting the following goals:
-
Musculoskeletal training
-
Neuromuscular Training
-
Cardiovascular Health
-
Balance Training & Falls Prevention
-
Cognitive Training
-
Functional Mobility

Participants exercises and activities are developed general training recommendations for different functional levels:
-
Dependent - These individuals require one-on-one, hands-on assistance for most activities. They are not candidates for circuit training or group-based exercise.
-
Frail - These clients have low functional capacity across several areas.
-
Independent -This describes the “typical” older adult
-
Fit - Fit older adults are physically active but may have gaps in certain areas
-
Elite - These are highly active, high-functioning older adults
This treatment appraoch bridges the gap between physiotherapy and independent activity.
How is Safety Maintained During Circuit Training?
Circuit training is highly effective for building strength, balance, and confidence—but safety is always our top priority. To protect older adults and reduce fall risk, we structure every session with:

-
Close supervision for frail or higher-risk clients
-
Balance support at each station (chairs, bars, or handholds)
-
Clear, simple instructions for every exercise
-
Ongoing physiotherapist attention to technique and fatigue
-
Unstable surfaces used only when safely supervised
-
Education on monitoring effort levels with RPE or the talk test

This ensures every client trains safely while gaining the most benefit from each session.
